Payment and Fees
Insurance company’s willingness to cover these evaluations varies from one company to the next, as well as one situation to the next. Individuals seeking one-time evaluations are responsible for checking with their insurance company and understanding their out-of-pocket responsibilities.
The fee structure for direct/private pay one-time evaluations varies by clinician. The ranges are listed below:
- $300-500 per evaluation (includes: 45 – 90 minutes face-to-face time, record review, and report writing — up to 6 hours of total work)
- $50-100 per hour after the first 6 hours of total work
In accordance with ethical, legal, and industry standards, fees paid are for the evaluation service and qualified medical opinion. We cannot guarantee specific diagnoses or other clinical conclusions, and payment is not contingent upon a specific outcome.
Payment is due, in full, prior to receipt of the final letter, report, or other evaluation product.
Process
If you wish to be seen for a one-time evaluation, contact Desert Wise at our main number or via our contact page.
Due to the length of time required to complete evaluative write-ups, all examinees are urged to schedule and complete their evaluation well before any related deadlines/suspenses.
We offer both in-person and virtual evaluation options – except in limited cases that require in-person testing.
You will be provided the required paperwork by administrative staff – who will also schedule you for your evaluation. Your paperwork must be completed in advance. You are expected to arrive at your evaluation appointment at its scheduled start time to ensure your evaluator has sufficient time to complete the evaluation.
You must bring any relevant mental health records you wish to have considered in your evaluation. If your evaluation pertains to specific information not readily available to the general public (e.g., PD, FOUO standards), bring a copy of this information to your evaluation. Note: Desert Wise evaluators are competent at acquiring DoD- and other federal-related guidance and standards.
Our clinicians will give you full and ample opportunity to provide all relevant information (i.e., both verbal and written) and guarantee and objective examination and report.
For all individuals self-referring for an evaluation, the final letter, report, or other evaluation product remains your sole property and will not be provided to any other party without your written consent.
Please reach out to our administrative staff with any additional questions or concerns.
